Reporting Hazards

All workers must report:

• Unsafe conditions or behaviours

• Near misses or incidents

• Broken or faulty equipment

Reporting procedure:

1. Inform your supervisor or site manager

2. Complete a Hazard Report Form

3. Assist in follow-up actions or investigations

REMEMBER: Reporting is about prevention — not punishment.
